If you want to report an issue to your organization's management, such as missing equipment, a maintenance issue, or a safety incident in the field, you can create and submit an issue in the Samsara Driver App through Forms. Create an issue directly through a digital form if it has to do with the item in the form. If you see an issue that might not have to do with a form, you can also create an issue adhoc, or without a form.
To create and report an issue to your organization's management, select the appropriate workflow:

Create an Issue in a Form: Creates an issue relevant to an inspection item within a form, such as missing equipment or a maintenance issue.

Create an Issue without a Form: Creates an issue relevant to a subject that may go beyond the scope of a form, such as a safety incident in the field.
To create an issue in a form about an inspection item, perform the following workflow:

If there is an open issue (
) on an item you were going to create an issue for, tap the open issue to confirm it is not a duplicate for what you were going to report.
The Samsara Driver App displays currently open or unresolved issues for an asset. This saves you the time of flagging an item if it a known issue.

Within the form, tap Add a Note below the item response.

Describe your issue and upload any supporting images, as needed.

Tap Create Issue.

Tap Done to submit your issue.
There may be instances where you need to create an issue without form, such as a safety incident in the field. To create an issue without a form, perform the following workflow:
